3 on. 3 2009 Registration Form

To enroll a team into the Soccer Tournament for 2009 you need to either:

Mail in Registration:
1) Click here and download the .txt form.
2) Open the document 3on3Registration.txt and fill out all information.
3) Mail registration form and check to Phil and Barb Whitefield.

Online Registration:
1) Click here and fill out the form.
2) Click send if all information is accurate.
3) Mail check to Phil and Barb Whitefield.
